Backgrounder: healthcare AI governance and ISMAI 2026
Healthcare AI governance concerns the policies, responsibilities, evidence standards, monitoring systems and institutional processes needed to use medical AI safely and accountably in clinical settings.
What healthcare AI governance means
Healthcare AI governance is the set of institutional arrangements that allow hospitals, health systems and professional bodies to adopt, monitor and, where necessary, withdraw medical AI systems responsibly. It covers policies, responsibilities, evidence standards, procurement, monitoring, incident learning, liability and clinical accountability.
Governance is not a substitute for validation or clinical judgement. It is the framework that keeps validation, monitoring, clinical decision-making and patient safety connected across the life of a medical AI system.
Why governance matters before clinical AI is scaled
Once a medical AI system is present in a clinical pathway, its behaviour affects referrals, triage, diagnosis, decision support and communication with patients. Institutions therefore need clarity, in advance, about who is responsible for what, what evidence is required, how performance is monitored over time and what happens if a system begins to underperform.
Without institutional readiness, a promising tool can be adopted faster than the local service can support it, and safety and equity may suffer as a result.
Governance questions discussed at ISMAI 2026
- What must be true of a service before a medical AI system is introduced?
- How are validation and post-deployment monitoring built into procurement?
- Where do clinical, institutional and supplier responsibilities meet?
- How are incidents recognised, reported, investigated and learned from?
- What are the mechanisms for restricting or withdrawing a system that is no longer safe?
